This will probably sound like a daft question to most...

I took my dog to eye test yesterday but managed to go without correct paperwork (first time I'm taking a dog in UK).
I had her foreign registration papers and UK three generation pedigree(green) with me.
I was told I need to send them the original Registration document , simple I thought so didn't ask to see an example of what they mean.

I have gone through all the paperwork I have for my dog and only thing I can thing this -Registration Document-  is titeled Owner Registration Certificate.
It is a browny orange in colour and details current registered owner and my dogs details...
On the other side it has  application for a change to the registered ownership form.

Would this be the document they need?
I can obviously call them tomorrow but I thought somebody here might be able to confirm this is/is not the document I need so I can sit down, breathe and stop searching :-)

I had slight panic creeping in as I booked her for hip xray and they told me to take this same document with me... The document I satrted to think I have lost!
I have been saying I really must put all her docs together in a folder.. Lesson learned and a folder will be bought tomorrow :-)
